I think it's more to do with the fact they weren't as thorough in those days: They didn't used to tie off the ends, meaning it could heal and reattach.

My memory is that Fin said something like "Sophie told me you had problems with your first pregnancy" (even though Sophie wasn't around at the time either, but never mind).

When did Pippa and Tom adopt Sally? Why did she keep her name as Fletcher instead of change it to Ross?

She was adopted by Tom and Pippa so legally her surname became Fletcher. She was never adopted by Michael.

It happened in 1989, I think after everything that happened with Dodge, when Tom and Pippa wanted to make Sally feel like she was really part of the family.
