Yeh, I just realised that and edited that out my last post

I don't know why, but to me it looks like "fist to" is crossed out. As if she was going to say " with his fist to his face" instead of " With his hand to his face"

Its something like "holding his f***to hand" or thinking outside the box it could origanally have been "face to hand" and got changed to "Hand to face" maybe because face to hand sounds more like face in hands (grieving/upset) and hand to face is more of a hiding movement. Maybe the initial thoughts was that it was the upset grieving gesture but looking back the nurse thought it must have been a hiding one.

Edited because I don't know what the hell a "hoding movement" is but it sounds like something done by gangs of lads who hang around street corners at night and wasn't quite what I was aiming for.
