I assumed it was the reason why the mother couldn't look inside the coffin

She threatened to resign after Chris suggested that Ingrid and Justine run two separate surgical teams. Chris of course promptly accepted her resignation and offered her job to Justine. Ingrid's last scene was her clearing out her desk. Justine walks in and Ingrid tells Justine exactly what she think of her, to which Justine retorts, by pointing out that she has a consultant's job and Ingrid has nothing. Ingrid must be kicking herself for insisting on hiring Justine.
